Selecting and preparing forms that affect the rights of others is considered the practice of law under Rule 24 of the Washington Supreme Court's General Rules (GR 24). Limited Practice Officers (LPOs) are authorized to select and prepare forms approved by the Limited Practice Board within the limitations of the LPO license. LPOs are not authorized to advise individuals on the use of these forms. Anyone seeking advice or assistance to fill out such forms should contact a legal clinic or lawyer.
Section 1 – Mortgage Lending Documents

Section 5 – Farmer’s Home Administration Forms
All documents approved and designated for use by the United States Department of Agriculture Farmer’s Home Administration in connection with closing a FMHA Loan on residential property.
Cover Page Creation Process
The first step in creating a Cover Page is selecting the Document Type from the drop-down list at the top of the Document Tab. The drop-down list includes the names of all document types recorded/filed by the City Register. Document types appear in the drop-down list in alphabetic sequence.
The Property Tab is used to enter information identifying all properties associated with the document submitted for recording.
The Parties Tabis used to identify all parties to the document. Click the radio button to select the type of Party Name. Party labels change depending upon the document selected.
ACRIS Cover Page will perform tax calculations for:
NYC Real Property Transfer Tax (RPTT)
NYS Real Estate Transfer Tax (RETT)
Mortgage Recording Tax (MRT)
Recording Fees for the document are automatically calculated based on the selected document type and information entered on previous screens.
After printing the Cover Pages for the current document, the Transaction screen is presented. Details for the current transaction are displayed.
If all documents included in the transaction have been completed, the transaction can be finished and the Payment Cover Page printed.
